New Patient Registration

In order to register with the Practice, please speak to a receptionist who will give you appropriate advice and the appropriate forms for completion.

You will be registered with the Practice, and every patient will be given a named GP on registration. Whenever possible, appointments will be arranged with the doctor of your choice.

On Registration

Please make an appointment for a new patient check. There is often a delay in receiving your medical notes and we prefer to be able to discuss your immediate needs as soon as possible. New patient health checks are provided to all new patients over the age of 5. If you are on any regular medication, you will be required to see the pharmacist or a GP prior to these medications being available to you on repeat prescription.

Change of Address

Please tell us when you change your address. You may tell us in person, by 'phone, by letter, or by fax (01223 844569).
